Homocysteine Supplement

My homocysteine level was 10.2, after 6 months on Methylcobalamin it is 11.3 I am new to this and my physician only said to add the methylcobalamin.
I am MTHFR positive one variant.
Dr. Bredesen advises combination of Methylcobalamin, P5P, Methyl folate in the active form of tetrahydromethyl folate.
I'm researching supplements but not sure of the correct amount of each in milligrams. Brands vary which makes it more confusing. .

I wanted to welcome you to the ApoE4 website! TinCup gave a great suggestion with the MTHFR wiki. In that wiki it does discuss some Nutritional Support for Methylation - its a little more than half way done on that page. Hi wlbox, In The End of Alzheimer’s pg 176-177, these are recommended doses of the B’s if homocysteine is above 6 :
~pyridoxal-5-phosphate (P5P) vitamin B6, 20 to 50 milligrams each day
~Methylcobalamin and adenosylcobalamin forms of B12 1 milligram total each day
~Methylfolate starting at 0.8 milligrams (and as high as 5 milligrams ) each day

Those are general recommendations. If your levels of B6 and folate are already optimal, maybe you only need the B12 (?)
If you have a copy of The End of Alzheimer’s there are further recommendations on those pages of what to do if the B vitamins alone don’t bring homocysteine down. (Adding TMG supplementation and reducing methionine in the diet). In his second book The End of Alzheimer’s Program pg 188 Dr Bredesen also states that “inadequate choline can also put you at risk of elevated homocysteine.”

welcome wlbox! I have pesky homocysteine as well. I've found success in taking extra methylfolate along with good amounts of B12. i was in the 9's but then took 2-3 grams of methylfolate per day and saw it drop in to the 7's. But I recently cut back on the amount of methylfolate and my homocysteine went back up. I think I'm drinking too much coffee too so both of those probably led to the increase. Back to less coffee and more methylfolate. Hopefully I can get this tiger tamed!

Wish I could find a supplement that contains what Dr Bredesen recommends.

The closest match I have ever found is in the LifeSeasons ReCODE protocol MultiVitamin. (and a bunch of other things, of course, since it is a multi.)
It has 800 MCT Methylfolate, 15 mg B6, and 1 gr B12, coming from 500 mg each of the methylcobalamin in and adenosylcobalamin.

There are a lot of methyl B vitamin supplements on the market but I haven’t found others that contain both forms of B12.
One example is Thorne Methyl-guard Plus. I am not recommending it but is it one I have taken it in the past when my homocysteine was high. if you take 2 rather than their recommended 3 caps , you get 30 mg P-5-P, 2 mg folate, 2 mg methylcobalamin. So it is higher on all of them than Bredesen protocol . If you take 1 cap, you’d get 15 mg p-5-p, 1 gr folate and 1 gr B12. But none of the adensyl- form. Thorne formulation also has TMG, .5 gr per cap.
In my own case, my homocyeine responded better to choline supplementation than the methyl B’s, so I stopped taking methyl B supplement.
It all depends on your own unique biochemistry.
