Susan and Ron,
thank you for your comments and hints.
From what I gather so far, my high LDL ist not the only culprit and lowering it drastically with Statins as the main strategy is questionable. I am going to try a low fat (mostly vegetarian) diet, with saturated fat sharply reduced and take high grade fishoil 2 g per day (EPA). EPA seems to be more important than DHA for the heart, although we need both.

If Trigs/HDL ratio is important, mine is around 2, therefore far away from 4. So I should be able to relax a bit.

Still I need to reduce that plaque in my carotid artery and I am thinking of the "Track your plaqe" program of Dr. William Davis. Has anybody tried this?

Thanks again.

Thank you, nice new paper putting LDL and statins in perspective. I found it interesting that they used coronary artery calcium:
... to better define risk as recommended in the guidelines.1

1. Grundy SM, Stone NJ, Bailey AL, et al. 2018 AHA/ACC/AACVPR/ AAPA/ABC/ACPM/ADA/AGS/APhA/ASPC/NLA/PCNA guideline on the management of blood cholesterol: a report of the American College of Cardiology/American Heart Association Task Force on clinical practice guidelines.

Somewhere I got the impression that the 'officials' didn't recommend CAC as somehow not reliable enough. I must have been mistaken.

I also just found this, re Tg/HDL ratio, not exactly sure if it means that the ratio only applies to women;

In a Cox proportional hazard model stratified by statin use adjusting for confounders, among those not on baseline statins, triglycerides were independently predictive of major adverse cardiovascular events in women (per 50 mg/dL risk ratio: hazard ratio 1.25 [95% CI: 1.06, 1.47]; P=0.01). This was not true among men. The interaction between triglycerides and sex, and triglycerides and statin was statistically significant.

My own ratio is 0.64 using European mmol, well under the 0.97 or so he recommends. However my LDL is 4.47 (Friedwald)/165, so things may get tricky if the ratio is meaningless...

From that article, it's not clear how many individuals were harmed by achieving their target LDL values, I only see mention of a few deaths that were prevented.

SusanJ, please let us know if healing your gut helps your LDL. I am also working on both issues, and I will try to see if it helps me too.
